The Dodge County Sheriff's Office has posted several forms which may be of assistance to the public in addition to the deputies. The forms listed below may be downloaded and then printed. You may be asked by deputies to print a form from this page and mail it to the sheriff's office. It will make dealing with paperwork more streamlined and efficient.

Information for Victims of Crime in Wisconsin — The Dodge County Victim Information and the Victims of Crime in Wisconsin forms are given to victims of crimes. They contains much needed information related to victim's rights, crime victim compensation, restitution, etc. Important phone numbers are also listed on these form.
No Consent Form — The No Consent Form is signed by victims of crimes saying they did not give consent for the crime to occur. This form is needed for successful prosecution of charged individuals. The form has three parts:
- Burglary
- Theft/Criminal Damage to Property/Operate without owners consent (stolen autos)
- Battery
When asked to do so, please fill out the appropriate portion of the form. Please be as detailed as possible. If you have questions, please contact the deputy who asked you to complete the form.
Voluntary Crash Statement Form — The traffic accident statement form is used by investigators to determine what occurred before, during and after a traffic crash. When asked to complete, please fill out the form as thoroughly as possible. This is a two page form so please complete both pages.
Driver (Self) Report of Traffic Accident (Wisconsin DOT Form MV4002)
Voluntary Statement Form — The voluntary statement form is used by victims, witnesses and other involved people. When asked to complete, please fill out the form in as much detail as possible. Please print and complete as many pages as necessary.
NCIC Stolen Property Worksheet Form — The National Crime Information Center (NCIC) requires that this form be completed and signed by the owner of an item that was stolen. Once this form is completed, the item may be placed into the NCIC computer. Once the item is entered, if the item is queried by a law enforcement agency, the item will be shown as stolen. This query can be done nationwide. When asked to complete, please print this form and fill in all the blanks possible and sign the bottom.
Missing Person Certification Form — The National Crime Information Center (NCIC) requires that the law enforcement agency have in its possession written documentation certifying that certain conditions exist, prior to entering a missing person into NCIC records.
Authorization for Disclosure of Health Information — The Authorization for Disclosure of Health Information Form must be signed before the sheriff's office can obtain such records. When asked, please fill the form out completely and forward it to the sheriff's office.
Community Program/Appearance Request — The Dodge County Sheriff's Office does frequent talks and demonstrations to public groups. If you would like the sheriff's office to appear for your organization, club or school, please download this form and complete. The form should be sent to the sheriff's office and someone will contact you.
Community Program/Appearance Evaluation — The community program/appearance evaluation form is used by the sheriff's office to evaluate the effectiveness of the programs. Information obtained from the evaluations is used to improve the quality of the presentations. We request that the form be completed and forwarded to the sheriff's office after a presentation.
Business/Residential Information Form — The business/residential information form is completed by business owners and homeowners and forwarded to the sheriff's office. The sheriff's office uses the information obtained to contact keyholders in event of an emergency. Having this information will make it much easier and faster to contact business owners or homeowners. Please complete this form and forward it to the sheriff's office.

Citizen Observation Waiver - Communications
The Dodge County Sheriff's Office Patrol Division and Communications Division periodically allow citizens to participate in ride along/observations. Those applying for a ride along/observation will be subject to a background check and be required to meet specific criteria and approval from the Patrol Captain or Communications Lieutenant.
