INSTRUCTIONS FOR STATION EMPLOYEES

First determine if the drive off was an intentional criminal act, and then ask yourself the following questions:

1. Was there no credit card swipe attempted on a properly working credit card reader at the pump (verified)?
2. Did the act involve evasive behavior such as the gas hose being set on the ground, the license plate missing from the vehicle, etc?
3. Was the person(s) told to stop and did not? Did they run from the store? Did the vehicle speed away, etc?
4. Did the person taking the gas commit other illegal acts such as retail theft, etc?

If the above criteria is met, and you believe this was an intentional drive off, please submit the form below and a deputy will contact you. 

An unintentional gas drive off is a civil matter and cannot be handled by an officer.

If there is no obvious criminal intent, this is a civil matter:

• Vehicle owner information cannot be given to you by law enforcement.
• To legally obtain the vehicle's registered owner information, you must complete the state form MV2896. Click here to obtain the form from the state.
• Mail the MV2896 form to the Wisconsin DOT at the address on the form along with the fee. Law enforcement has no legal authority to collect money or order payment in any civil matter. The business may correspond with the registered owner of the vehicle in an attempt to collect payment for the gasoline or may pursue civil action.
